I need a template so that I can just plug in names and maybe a few comments to send home with my students who aren’t doing their work or are failing due to other circumstances. Anyone know where I could find such a template?

i don’t think a template is needed, just e-mail the students parents

Try this site. They state “You can modify most of the documents found at the sites discussed in this section before printing them…” They have quite an extensive amount of material available.
Also most on-line grading systems as in “Little Red Schoolhouse” or “MyGrade”Book” etc. have templates available for teachers to use when sending out grade reports.

just type your own on microsoft words leaving in blanks (______________) and make yourself a rubric, like for example:
1 Not completing work
2 Talks too much
3 in danger of failing
etc, etc
then just fill in the students names and write in the numbers
also remember to write good notes for those students who are doing good in class, that’ll surely encourage others to do better
